Claims

While providers typically submit claims on your behalf, you may be required to pay for services out-of-pocket in certain situations. In such cases, you must complete and submit a claim form to receive reimbursement from your insurance company.

How To Submit Claims For Reimbursement

In order to process your claim, you’ll need to provide UnitedHealthcare with the following.

    • Itemized Bill
    • Proof of your payment such as a receipt
    • Complete a claim form.

Once you have that information, you’ll need to submit those documents through UHC’s My Account student portal. You can monitor the progress of your claim through My Account.
